refactor: reorder toolbar buttons for better UX

- New order: User, Home, Settings, Add, Refresh, Logout
- Profile/user first for identity prominence
- Core navigation (Home, Settings) before actions
- Actions (Add, Refresh) grouped together
- Logout at the end as final action
